How should students who never left Ithaca complete the Re-entry Checklist?

Dear Deans,
I have a question about the Re-entry Checklist, which we were recently informed all students must complete. I’m unable to complete the checklist as it requires answers to questions about my arrival in Ithaca, such as which state I will be coming from and whether I will be able to get tested before arriving. I can’t answer these questions because I am already/still in Ithaca, like many other graduate students who have been in Ithaca throughout the pandemic. Unfortunately there doesn’t seem to be an option for us to indicate that on the checklist. Could you advise as to how students in my position should complete the checklist? Many thanks in advance!

Thanks for your question and for trying to complete the Re-entry Checklist correctly. If you have questions about the Re-entry Checklist, or about the Daily Check once you are required to complete that online form, contact Cornell Health at 607-255-5155.
Regarding your specific quandary, if there is an option to indicate that you will be “coming from New York State” on the checklist, that may be the best way to handle that question. Cornell Health should be able to provide definitive guidance.
