Current graduate students can find all Graduate School forms on this page, sorted by category. For assistance with forms or to request the PDF version of an online form, please contact gradstudserv@cornell.edu.
PDF forms should be downloaded, completed, and submitted by email. Adobe Reader can be downloaded free of charge. Online forms should be completed and submitted via the respective links.
Once a form has been submitted, the Forms Portal will allow students and faculty approvers to view the status and history of any submitted electronic forms.
